

SAGE Upstate promotes the well-being of older gay, lesbian, bisexual, transgender, queer & questioning people in CNY through health programs, socials, support groups, & education.


Take Care Together
When you have support and health strategies, healthy aging is possible. Building friendships while making life choices improves your outlook and your health. The Take Care Together program aims to understand how countering isolation with safe affirming programs leads to healthier lives. This program was made possible by funding from Excellus BlueCross Blue Shield.​
SAGE Upstate goals: Reduce Isolation and Improve Health for older LGBTQ people.
Isolation impairs these goals.
Isolation may lead to poor nutrition, depression/anxiety, frailty and delayed care for health conditions.
In addition, LGBTQ older adults face other challenges: we are less likely to have children to rely on for support,
more likely to live alone, and wary about reaching out to mainstream services.

How To Participate In Take Care Together
Register by taking a pre-program survey here.

From March - May 2025 Participate Monthly In:
2 social programs
1 health education program
1 fitness program per month.​
1 discussion group about your participation -- in person or on Zoom
Over the Summer
Continue participation in the frequency and on the topics that make sense for you.
Plus -- participate in a one discussion about the impact of these programs -- in person or on Zoom.
These will be held in June, July and August.

In September
Join us for a closing dinner to discuss the findings of the program.
